What documentation do I need to open a business account at Manasquan Bank?

Sole Proprietor

Documentation Required:

• If using your own Social Security Number, provide Tax Return 1040c
  • If not, provide proof of EIN:
    • SS-4 or 1st Page of Current Tax Return
• If a trade name is used, provide a copy of the Business Name Certificate (DBA Certificate)

Limited Liability Company (LLC)

Documentation Required:

• Proof of EIN:
  • SS-4 or 1st Page of Current Tax Return
  • If using your own Social Security Number, provide Tax Return 1040c
• Copy of Articles of Organization or Certificate of Formation
• Copy of Operating Agreement (If available)
  • Operating Agreements are not required for a Single Member LLC

Corporation

Documentation Required:

• Proof of EIN:
  • SS-4 or 1st Page of Current Tax Return
• Copy of Certificate of Incorporation
• Copy of Corporate Resolution or Bylaws
• If the Corporation is operating under an assumed name/DBA provide a copy of the Assumed Name/Fictitious Name or Business Name Certificate

Non-Profit

Documentation Required:

• Proof of EIN:
  • SS-4, 1st Page of Current Tax Return or Tax Exemption Letter
• Copy of Certificate of Incorporation
• Copy of Resolution/Bylaws
• Copy of Meeting Minutes
  • Should state the authorized signers for the bank account
